Freedom Home Care Acquires Elder Care Services
Expands scope services to include Case Management
MANKATO, Minn. (August 1, 2017) – Freedom Home Care, a locally owned group providing non-medical home care services, is pleased to announce the acquisition of Elder Care Services. The operations of Elder Care Services will be immediately merged into Freedom Home Care’s with Sharon Chader, founder and owner of Elder Care Services, joining the staff and leading Freedom Home Care’s new Case Management service line.
“The concept of providing both Case Management and traditional Home Care services within our agency has been a vision from the very beginning of Freedom Home Care,” says Chris Mihm, co-owner of Freedom Home Care. “We believe this model will allow us to serve more clients, and most importantly to make sure their needs are being fully met.”
Elder Care’s staff brings a breadth of experience to Freedom Home Care. A well-regarded leader in providing exceptional care, Elder Care Services was founded in 1996 by Chader, a Licensed Social Worker and Certified Care Manager.
“We have highly respected the care Elder Care Services has provided for many years and admired their passion for working with clients to maintain their independence and vitality,” says Christine Nessler, co-owner of Freedom Home Care. “With their added expertise and experience, we will strengthen our team and be able to continue to bring to our communities their unique model of Case Management services.”
Freedom Home Care’s new Case Management services will include identifying and coordinating resources and services, coordinating and attending appointments, relocation assistance, 24/7 emergency availability, crisis intervention, assessments and consultations, advocacy and education, assisting with health care advanced directives, acting as a Health Care Agent and providing Case Management reporting.
“While I am not ready to retire from Case Management, I am retiring from the many daily responsibilities of owning a business,” says Chader. “I founded Elder Care Services on a philosophy of providing the best possible care and continuity for our clients through their many life changes. I am pleased to know that through this acquisition that legacy of true care and peace of mind will continue.”
