Ann Cesafsky, LSW, has dedicated her career to partnering with older adults and their families to help navigate the many transitions accompanying aging. Her passion for serving seniors began during her college years and has grown into 16 years of experience in elder care, home care, and case management.

 A graduate of the Minnesota State University, Mankato Social Work Program, Ann completed internships and field placements in a hospital setting, a skilled nursing facility, and community-based senior programs, including working with the Retired and Senior Volunteer Program (RSVP). These early experiences established a strong foundation in person-centered care and advocacy.

Throughout her career, Ann has worked with organizations including Elder Care Services and Freedom Home Care, where she provided comprehensive case management and care coordination for older adults wishing to remain independent while receiving the support they need. She takes pride in building long-term, trusted relationships with clients, their families, healthcare providers, and community partners. Her philosophy is to work alongside each client as an advocate, ensuring their goals, preferences, and quality of life remain at the center of every care decision.

Ann has extensive experience helping families navigate complex healthcare systems, coordinate services across multiple care settings, and plan for changing needs.She’s experienced in educating clients and families on dementia, caregiving, grief, advanced care planning, and person-centered care helping families better understand the challenges of aging and caregiving. 

She’s also worked with  guardianship services, gaining valuable experience advocating for vulnerable adults and protecting their best interests. Passionate about dementia care, Ann completed specialized training through the Alzheimer’s Association and previously co-facilitated an Alzheimer’s Association affiliated caregiver support group. As a former board member of the Chesley Board on Aging she gained insight into the community’s needs, the importance of policies and educational programs. She has also presented to Social Work classes and was a panelist at the 50th Anniversary of the Social Work Department at Minnesota State University in 2024.
