The Back to Home Care Program is Designed to Help Make a Transition Safe

Transitioning back home after rehabilitation or hospitalization can help prevent re-hospitalization and/or readmission to a skilled nursing facility. Do you or a loved one:

  • Take many medications or had former medication errors
  • Have multiple chronic conditions
  • Make frequent trips to the emergency department
    or hospital
  • Prefer to remain in own home
  • Have difficulty getting to doctor appointments
  • Have balance issues

Freedom Home Care wants to ensure a safe return home by offering an affordable short-term package of care designed to help you with transitioning home. Assisting in making return visits to the doctor, following therapy exercise protocols and managing new medications gives you more peace of mind as you recover from your surgery, injury, illness or fall.

 

The Back to Home Care Package Includes:

  • Five, eight hour shifts with qualified caregivers
  • 20% off regular hourly home care and case management fees during the first five days of service
  • One month 24-hour emergency contact service*
  • One FREE nurse assessment including a home safety check, coordination of care, assessment of client condition and needs and 72-hour follow up with hospital case management

Day One

  • Assistance packing belongings from hospital or rehab
  • Transportation home by caregiver

  • Review of discharge plan

Days Two to Four

  • Bathing and toileting assistance
  • Personal cares

Day Five

  • In-home care previously mentioned
  • Follow up appointment with primary care provider with case manager
  • Summary of care and recommendations from RN
