Home Nursing Services for Exceptional Care

Our team of registered nurses can assist and provide clients with a variety of services including:

 

Medication management: Our registered nurses are able to check that our clients are taking their prescriptions and other drug treatments correctly as well as set up their medications on a regular basis.

 

Hands-on assistance with transfers and mobility: Caregivers are trained in transfer techniques to help our clients that are using wheelchairs, walkers and gait belts to safely move about their home environment to reduce the risk of falls.

 

Delegate nursing tasks to caregivers: Our registered nurses train our caregivers on the skills needed to perform daily cares and transfers of our in-home clients. They also train our caregivers on any task needed for a client’s specific care needs that may be unique.

 

Coordination of Care: The Freedom Home Care team of case managers (registered nurses and licensed social workers) will provide open communication between our clients, doctors, facility social workers and any other approved member of the client’s care team.

 

Regular Monitoring Visits: Clients receive regular monitoring visits from our nurses. Nurses will check to see if the client’s care needs have changed by noting the client’s diet, drinking, vitals, pain levels, home safety and the success of the current plan of care.
